Flight Attendant Oxygen Generator Update

Date: September 26, 2026 AFA is providing this communication to ensure our members are aware of a newly identified safety concern involving the E-175 Flight Attendant Oxygen System. On September 24, 2026 a hazard was identified during testing of our Flight Attendant oxygen system/s on one of our aircraft. During the test, a galley crew oxygen mask failed to activate during an accessibility check. This happened becuase the generator trigger pin did not disengage when the mask was pulled, as required. Inspection found the oxygen generator was mis-aligned causing the cable and pin to pull at an angle rather than straight out. Three of the four inspected generators were mis-aligned on this aircraft. Subsequently, additional checks were made on more aircraft. Some of these aircraft were found to have the same issue. To further explain, there are four Flight Attendant oxygen generators on our aircraft, one at each of our aircraft doors (L1, R1, L2 and R2). Any of these, or all. may have the same mis-aligned configuration, which may affect the ability of Flight Attendants to receive oxygen in the event of an emergency or rapid decompression. CURRENT SAFETY CONCERN AFA's concern is whether a Flight Attendant positioned in their jumpseat, or working in their galley, would have their designated oxygen available if an emergency or rapid decompression occurred. Our ASHS Chair has also discussed this concern with our FAA Safety Inspector. And both our ASHS Chair and our MECP are continuing to work through this critical safety issue with the Safety Department at our AFA-CWA International Office, and Horizon Air senior leadership.
